Job summary

CenterWell Home Health in Florida is seeking a Home Health Occupational Therapist Assistant to support patients in regaining strength and independence at home. Working under a licensed Occupational Therapist, you will help deliver therapy services, adapt environments, and coordinate care with the interdisciplinary team to promote functional recovery.

Preferred candidates hold OTA licensure, CPR, and a valid driver’s license with reliable transportation.

Qualifications

Current and unrestricted OTA licensure.
Current CPR certification.
A valid driver’s license with auto insurance and reliable transportation.

Responsibilities

Provide therapy services planned, delegated and supervised by the Occupational Therapist.
Assist in the implementation of vocational/education programs and activities established by registered Occupational Therapist.
Design/adapt equipment and living environments; fabricate devices to assist and improve function and independence.
Provide therapeutic treatment to improve strength, coordination, range of motion and function.
Document observations in patient records and prepare progress reports.
Instruct patients and family members on home programs and use of adaptive equipment.
Participate in care coordination and discharge planning with the interdisciplinary team.
Coordinate community reintegration activities to support physical reconditioning and psychological adjustment.
