Job summary

CenterWell Home Health is seeking a Home Health Occupational Therapist Assistant to help patients regain strength and independence in their homes, following a Plan of Treatment set by the supervising Occupational Therapist.

You will assist in program implementation, design/adapt equipment, and provide therapy while documenting progress and coordinating with the care team. A valid OTA license, CPR certification, and a valid driver’s license are required; home health experience is a plus.

Qualifications

• Current and unrestricted OTA licensure
• Current CPR certification
• A valid driver’s license, auto insurance, and reliable transportation

Responsibilities

• Provide therapy services planned, delegated and supervised by the qualified Occupational Therapist
• Assist in implementing vocational/education programs designed by the Occupational Therapist
• Design/adapt equipment and environment to restore function and independence
• Provide therapeutic treatment to improve/restore strength, coordination, ROM and function
• Document observations in patient records and prepare progress reports
• Instruct patients and family members regarding home programs and use of adaptive equipment
• Participate in care coordination and discharge planning with the interdisciplinary team
• Design community reintegration activities to aid physical reconditioning and psychological adjustment
