Job summary

CenterWell Home Health is seeking a Home Health Occupational Therapist Assistant to support patients in their homes under the supervision of an Occupational Therapist. You will help patients regain strength, mobility and independence as part of a patient-centered rehabilitation team.

Responsibilities include implementing treatment plans, documenting progress, and guiding families on home programs and adaptive equipment to enhance daily living and functional outcomes.

Qualifications

• Current and unrestricted OTA licensure.
• Current CPR certification.
• A valid driver's license, auto insurance, and reliable transportation.

Responsibilities

• Provide therapy services planned, delegated and supervised by the Occupational Therapist.
• Assist in implementing programs to restore and improve function.
• Design/adapt equipment and living environments to improve independence.
• Provide therapeutic treatment and instruction under the direction of the Occupational Therapist.
