Job summary

CenterWell Home Health is seeking a licensed Occupational Therapist to join our patient-centered team. You will assess daily living skills, design retraining programs, and help patients regain strength and independence in their homes.

Qualifications

• Licensed Occupational Therapist with current unrestricted licensure.
• CPR certification required.
• Valid driver’s license and reliable transportation.

Responsibilities

• Assess and screen patients' daily living/work-related skills with measurable goals.
• Develop and supervise therapeutic retraining programs to improve strength and function.
• Consult with physicians and rehab team to update the treatment plan.
• Provide therapy in patients' homes per physician orders.
• Document care observations, interventions, and progress accurately.
• Coordinate care and discharge planning with the interdisciplinary team.
• Design adaptive equipment recommendations to enhance home participation.
• Educate patients and families on home exercise and safety.
