CenterWell Home Health in Griffin, GA seeks an experienced Occupational Therapist to deliver in-home rehabilitation focused on restoring strength, mobility and independence. You will assess functional status, develop retraining plans, collaborate with physicians, and document progress for each patient.

The role requires an active OT license, CPR certification, and a valid driver’s license with reliable transportation.

Formación

• Current and unrestricted OT licensure.
• Current CPR certification.
• Valid driver's license and reliable transportation.
• Minimum six months of occupational therapy experience.
• Home Health experience preferred.

Responsabilidades

• Assess and screen patients' daily living and work-related skills and develop therapeutic retraining programs with measurable objectives.
• Confer with physicians and care team to develop or revise the Plan of Care.
• Provide therapeutic treatment to improve strength, coordination, range of motion and function.
• Document patient progress and care observations accurately and promptly.
• Supervise and train OTA and Home Health Aide performance in delivering therapy services.
• Coordinate discharge planning and care management in collaboration with the interdisciplinary team.
