• Confer with physician and rehab team to develop Plan of Care.
• Provide therapy to improve strength, coordination, ROM and function.
• Review and adapt retraining programs to meet goals.
• Consult with vocational team to create plans matching patient needs.
• Document patient status and progress accurately.
• Participate in care coordination and discharge planning.
