Job summary

CenterWell Home Health in the United States seeks a licensed Occupational Therapist to work in patients' homes under the Home Health Branch Administration. You will assess daily living skills, develop retraining programs, and administer functional tests to guide care plans, with focus on restoring strength, mobility, and independence.

Collaborate with physicians and the rehabilitation team, document progress daily, participate in discharge planning, and contribute to community reintegration.

Qualifications

• Current and unrestricted OT licensure.
• Current CPR certification.
• Valid driver's license, auto insurance and reliable transportation.
• Minimum six months of occupational therapy experience.
• Home Health experience.

Responsibilities

• Assess and retrain patients in daily living and work-related skills.
• Administer and interpret functional tests to guide Plan of Care.
• Collaborate with physicians and care teams to develop the Plan of Care.
• Provide therapeutic treatment to improve strength, coordination, ROM and function.
• Document care observations and progress daily.
• Participate in discharge planning and care coordination.
