Job summary

CenterWell Home Health is seeking a dedicated Occupational Therapist to deliver home-based rehabilitation, assess daily living skills, and collaborate with a multidisciplinary team to enhance patients' independence and quality of life.

You will design retraining programs, document progress, and participate in care planning and discharge activities to ensure optimal outcomes for individuals recovering at home.

Qualifications

• Current and unrestricted OT licensure.
• Current CPR certification.
• Valid driver's license, auto insurance and reliable transportation.

Responsibilities

• Assess/screen patient's daily living/work-related skills and develop therapeutic retraining programs with measurable objectives.
• Confer with the patient's physician and rehabilitation team to develop or revise the Plan of Care.
• Provide therapeutic treatment to improve/restore strength, coordination, ROM and function per physician orders.
• Review retraining programs and adapt to patient goals within the rehab program guidelines.
• Document patient status, interventions and evaluation in day services records and coordinate with care management.
