Job summary

Premier Home Health Providers Inc. is seeking a compassionate Occupational Therapist to help patients regain independence, improve function, and safely navigate daily living in the comfort of their homes.

You will evaluate home environments, create personalized treatment plans, and communicate with physicians and the care team to ensure coordinated, patient-centered care. This per diem role pays $130 per visit and offers a flexible schedule with potential opportunities to convert to full-time

Qualifications

• OT degree from ACOTE-accredited program.
• Registered with NBCOT.
• Active California OT license.
• Minimum 2 years of clinical OT experience.
• Current CPR certification (AHA).
• Valid driver's license and ability to travel to patient homes.
• Strong communication and organizational skills.

Responsibilities

• Evaluate patients’ functional abilities and home environments for safety.
• Develop personalized OT treatment plans and share with physicians.
• Teach and retrain patients in activities of daily living.
• Recommend adaptive equipment and orthotic devices.
• Maintain accurate clinical records and report changes.
• Coordinate with care team for patient-centered care.
• Educate family and caregivers on treatment approach.
• Supervise OT assistants and oversee home health aide care when OT is the sole skilled service.
• Support discharge planning as patients transition out of home health care.
• Perform other duties as required.
