Job summary

Premier Home Health Providers Inc. is seeking a per diem Occupational Therapist to help patients regain independence and improve daily living in home settings. You will evaluate home environments, develop personalized plans, and educate families while coordinating with a care team.

The role offers a flexible schedule, potential move to full-time based on performance, and mileage reimbursement for patient visits.

Qualifications

• Graduate of an occupational therapy curriculum accredited by the AOTA and AMA committees.
• Registered with the National Registration Examination of the AOTA.
• Active California Occupational Therapy license.
• Minimum 2 years of clinical OT experience.
• Current CPR certification with AHA.

Responsibilities

• Evaluate patients’ functional abilities and home environments to identify needs and safety concerns.
• Develop personalized treatment plans and communicate them to physicians and the care team.
• Teach and retrain patients in activities of daily living.
• Instruct patients on adaptive equipment, splints, and devices.
• Maintain accurate clinical records and report changes in patient condition.
• Coordinate with the broader care team for patient-centered care.
• Educate family members and caregivers on the treatment approach.
• Supervise OT Assistants and oversee home health aide care when OT is the sole skilled service.
• Support discharge planning as patients transition out of home health care.
