Job summary

VitalCaring, a home health and hospice provider, is seeking a Home Health Occupational Therapist to deliver patient-centered care in the Ocala area. You will evaluate function, develop goals, and work with families to restore independence in daily activities.

The role emphasizes collaboration with the care team, supervising COTAs, and participating in discharge planning to ensure seamless transitions for patients in their homes.

Qualifications

• Graduate of an accredited Occupational Therapy program.
• Current NBCOT registration.
• Current state licensure as an Occupational Therapist.
• Minimum of 1 year clinical experience preferred (home health a plus).
• Current CPR certification.
• Valid driver’s license and reliable transportation.

Responsibilities

• Evaluate patients’ functional status and daily living activities.
• Assess home environments for safety risks and recommend modifications.
• Develop individualized treatment plans with measurable goals.
• Provide therapeutic interventions to restore function and retrain daily living activities.
• Fabricate splints and train in adaptive devices as appropriate.
• Support patients returning to work with functional/work capacity evals.
• Educate patients, families, and caregivers on plans and ongoing care.
• Collaborate with care team to coordinate services and ensure patient-centered care.
• Participate in discharge planning for smooth transitions.
