Job summary

CenterWell Home Health is seeking a licensed Occupational Therapist to provide home-based therapy focused on rehabilitation and functional improvement. You’ll assess daily living skills, develop retraining plans and collaborate with physicians and the care team to optimize patient independence.

This patient‑facing role requires current OT licensure and CPR, a valid driver’s license, and at least six months of OT experience in home health preferred.

Qualifications

• Current and unrestricted OT licensure.
• Current CPR certification.
• Valid driver’s license, auto insurance and reliable transportation.

Responsibilities

• Assess/screen patient’s daily living/work-related skills and develop therapeutic retraining programs with measurable objectives.
• Confer with the patient’s physician and other health care team members and participate in development/revision of the Plan of Care Treatment.
• Provide therapeutic treatment and instruction to patients in accordance with physician orders to improve/restore strength, coordination, range-of-motion and function.
• Review/expand retraining programs in line with patient goals and rehabilitation guidelines.
• Design community reintegration activities to assist the client in physical reconditioning and coordinate with the interdisciplinary team.
• Accurately document care observations, interventions and evaluation; submit plans and discharge summaries to the supervisor.
