CenterWell Home Health seeks a licensed Occupational Therapist to evaluate patients in their homes and develop personalized retraining plans that improve daily living activities and functional independence. You will work with physicians and the care team to implement plans and monitor progress.

You will document outcomes, coordinate discharge planning, and support patient and family education. A valid OT license, CPR, and reliable transportation are required; home health experience is preferred.

Qualifikationen

• Current and unrestricted OT licensure.
• Current CPR certification.
• Valid driver's license, auto insurance and reliable transportation.

Aufgaben

• Assess/screen daily living/work-related skills and develop therapeutic retraining programs with measurable objectives.
• Provide therapeutic treatment and instruction to improve strength, coordination, range of motion and function.
• Document patient care observations, interventions and evaluation on day services rendered.
