Job summary

VitalCaring Group is seeking a compassionate Occupational Therapist (OT) for home health, full-time field-based position. You will assess patients, design and implement individualized plans, and collaborate with families and the care team to restore independence at home.

Qualifications

• Graduate of an accredited Occupational Therapy program.
• Current NBCOT certification and state licensure as an OT.
• CPR certification and valid driver’s license preferred.

Responsibilities

• Evaluate functional status and daily activity performance.
• Assess home environments for safety risks and recommend modifications.
• Develop individualized treatment plans with measurable goals.
• Provide interventions to restore function and retrain daily living activities.
• Educate patients, families and caregivers on plans and safety.
