Job summary

VitalCaring Group is seeking a dedicated Occupational Therapist for home health services. This full-time, field-based role focuses on helping patients regain independence and navigate daily living in home settings. You will work with families and an interdisciplinary team to tailor high-impact care plans.

Required are an accredited OT program, NBCOT certification, state OT licensure, CPR, and at least 1 year of clinical experience (home health preferred). A valid driver’s license is essential.

Qualifications

• Graduate of an accredited Occupational Therapy program.
• Current NBCOT certification/registration.
• Current state licensure as an Occupational Therapist.
• Minimum of one year of clinical experience; home health preferred.
• Current CPR certification.
• Valid driver’s license and reliable transportation.

Responsibilities

• Evaluate patients' functional status and daily living activities.
• Assess home environments for safety and recommend modifications.
• Develop individualized treatment plans with measurable goals.
• Provide interventions to restore function and retraining in daily living activities.
• Educate patients and caregivers on plans and safety.
• Collaborate with care team to coordinate services.
• Supervise COTAs and oversee home health aide care when required.
• Participate in discharge planning.
