Job summary

VitalCaring Group is seeking a dedicated Occupational Therapist (OT) for Home Health. This full-time, field-based role focuses on patient-centered care, evaluating functional status, planning interventions, and coordinating with families and the care team to restore independence at home.

You will work closely with patients, care teams, and families to implement individualized treatment plans, educate on safety, and participate in discharge planning to ensure seamless progress in the home setting.

Qualifications

• Graduate of an accredited Occupational Therapy program.
• Current NBCOT certification.
• Current state OT licensure.
• Minimum of one year clinical experience preferred (home health a plus).
• Current CPR certification.

Responsibilities

• Evaluate patients' functional status and independence.
• Assess home environments for safety risks and modifications.
• Develop individualized treatment plans with measurable goals.
• Provide therapeutic interventions to restore function and activities of daily living.
• Fabricate splints and train use of adaptive devices.
• Support return-to-work through evaluations.
• Educate patients, families, and caregivers on plans and safety.
