Job summary

VitalCaring Group is seeking a Home Health Occupational Therapist (OT) to help patients regain independence and navigate daily life at home. You will collaborate with patients, families, and interdisciplinary teams to deliver personalized care and measurable outcomes.

This full-time, field-based role emphasizes strong clinical judgment, excellent communication, and a compassionate approach to support safe transitions home and coordinated care across the care team.

Qualifications

• Graduate of an accredited Occupational Therapy program.
• Current NBCOT certification.
• Current state licensure as an Occupational Therapist.
• Minimum of one (1) year of clinical experience preferred (home health experience a plus).
• Current CPR certification.
• Valid driver’s license, reliable transportation, and auto insurance.

Responsibilities

• Evaluate patients’ functional status including mobility, strength, coordination, communication, and ability to perform daily activities.
• Assess home environments to identify safety risks and recommend modifications or adaptive equipment.
• Develop individualized treatment plans with clear, measurable goals focused on improving independence and quality of life.
• Provide therapeutic interventions to restore function, including retraining in daily living activities.
• Fabricate splints and train patients in the use of adaptive, prosthetic, and orthotic devices as appropriate.
• Support patients returning to work through functional and work capacity evaluations.
• Educate patients, families, and caregivers on treatment plans, safety, and ongoing care needs.
• Collaborate with the care team to coordinate services and ensure seamless, patient-centered care.
• Participate in discharge planning to support successful transitions and continued progress.
