Job summary

Palms-Home-Care is seeking an experienced Occupational Therapist to provide in-home health services focused on improving patient independence and safety within their home environment. The ideal candidate will have an active Florida OT license and background in home health, with strong documentation and time-management skills.

Responsibilities include in-home assessments, developing individualized treatment plans, assisting with activities of daily living, training on adaptive equipment,

Qualifications

Graduate of an accredited Occupational Therapy program.
Active Florida OT license.
Home health experience preferred.
OASIS experience strongly preferred.
Strong documentation and time management skills.
Valid driver’s license and reliable transportation.

Responsibilities

Conduct in-home evaluations and functional assessments.
Develop and implement individualized treatment plans.
Assist patients with ADLs (bathing, dressing, grooming, toileting).
Train patients on adaptive equipment and home safety modifications.
Complete OASIS documentation as required.
Collaborate with nurses, PTs, and other team members.
