Palms-Home-Care is seeking an experienced Occupational Therapist to provide comprehensive home health services focused on improving patient independence and safety within the home environment.

The role includes in-home assessments, developing individualized treatment plans, and guiding patients in activities of daily living. Strong documentation and Florida OT licensure are required, with preference for OASIS experience and home health background.

Competenze

• Graduate of an accredited Occupational Therapy program.
• Active Florida OT license.
• Home health experience preferred.
• OASIS experience strongly preferred.
• Strong documentation and time management skills.
• Valid driver’s license and reliable transportation.

Mansioni

• Conduct in-home evaluations and functional assessments.
• Develop and implement individualized treatment plans.
• Assist patients with ADLs (bathing, dressing, grooming, toileting).
• Train patients on adaptive equipment and home safety modifications.
• Complete OASIS documentation as required.
• Collaborate with nurses, PTs, and other team members.
