Job summary

Adara Home Health seeks an Occupational Therapist Case Manager to deliver skilled OT services in clients' homes within Rochester, MN. You will plan, implement, and reassess care per Plan of Care, reporting to the Clinical Supervisor and partnering with physicians.

You will travel to client homes 1:1, manage schedules during office hours, and contribute to a supportive team environment with opportunities for productivity bonuses.

Qualifications

• Licensed OT in Minnesota with no license restrictions.
• Graduate of AOTA-approved school and AOTA-certified.
• At least two years of healthcare experience; home care preferred.
• Strong communication, teaching, and independent-work abilities.
• Valid driver's license and access to reliable transportation.

Responsibilities

• Perform home assessments and plan/adjust care to meet goals.
• Provide OT treatments, evaluations, and progress notes.
• Translate exercises into ADLs and supervise equipment use.
• Instruct and supervise COTAs, HHAs, LPNs, and RNs when needed.
• Educate families and coordinate community resources.
