Aveanna Healthcare is hiring an Occupational Therapist for home health in the Winona, MN area. This salaried position offers daytime visits and a focus on one-on-one care to help patients live independently at home.

We provide a $10,000 sign-on bonus, and require an OT degree with a current MN/WI license, CPR, and a valid driver’s license. Experience with OASIS and home health is preferred. Join a mission-driven team delivering compassionate, quality care.

Qualifications

• Degree in Occupational Therapy from an accredited university/program.
• Current, unrestricted state license as an Occupational Therapist in WI and MN.
• Demonstrated proficiency in clinical assessments, documentation and compliance with policies and procedures.
• Valid CPR and Driver's License.

Responsibilities

• Perform initial and ongoing assessments to evaluate mobility, coordination, strength, and current level of function.
• Provide written evaluation and plan of care defining occupational therapy needs, goals, progress and outcomes.
• Deliver occupational therapy services per physician orders.
• Maintain plan of care with timely documentation and discharge planning as appropriate.
• Coordinate services with healthcare team and educate patient and caregivers on home exercise program.
• Supervise OTA and HHA as applicable and complete OASIS assessments where allowed by state.
