Job summary

Optum is seeking an Occupational Therapist in Home Health. You will assess and evaluate patient care needs related to functional status, ADLs, fine motor, home assessments and adaptive equipment as defined by medical conditions.

Key responsibilities include working within OT scope per state laws, coordinating with the health care team, evaluating functional status for CMS documentation, and reporting patient progress.

Qualifications

• Current Occupational Therapy licensure in state of practice.
• Current CPR certification.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client.
• Demonstrated ability to manage multiple tasks.
• Good communication, writing, and organizational skills.

Responsibilities

• Provides service within the scope of practice as defined by state laws governing Occupational Therapy and the plan of care.
• Evaluates the patient's functional status and CMS documentation; consults with physician on plan of care.
• Observes, records, and reports patient responses to treatment to supervisors and physicians.
• Conducts patient assessments evaluating function using functional ability tests.
