Job summary

Adara Home Health, Inc. in Minnesota seeks a licensed Physical Therapist to provide and oversee home health services, collaborating with clients, families, and the care team to develop goal-centered plans that consider quality and financial aspects.

You will travel to client homes for 1:1 care, manage a schedule during office hours, and provide education to clients and staff, reporting to the Clinical Supervisor.

Qualifications

• Two years of experience in a health care facility or home health care.
• Demonstrates critical thinking and clinical judgment.
• Ability to work independently with minimal supervision.
• Ability to build and maintain professional relationships.
• Excellent written and oral communication skills; teaching abilities.
• Comfort with basic computer skills; EMR and Outlook experience.

Responsibilities

• Reflects the company mission and values in care delivery.
• Assess clients to determine care needs and evaluate outcomes.
• Collaborate with physicians and home health team to update care plans.
• Ensure safety by assessing home environments and reporting concerns.
• Maintain documentation accuracy, timeliness, and compliance.
• Provide client and caregiver education; train staff as needed.
• Deliver direct care to clients across ages and conditions.
• Coordinate patient care with family and community resources.
• Provide clinical oversight to licensed and unlicensed staff.
• Manage financial aspects of home health services.
• Adapt and schedule flexibly to changing needs.
