Qualifications

• Current license to practice occupational therapy in the assigned state.
• Graduate of ACOTE-accredited OT program.
• Eligible to take NBCOT certification examination.
• One year related OT experience; regional clinical leadership approval required if less than 1 year.
• Current CPR certification.

Responsibilities

• Deliver occupational therapy services under physician-approved plan of care.
• Ensure plan of care addresses problems identified in assessment and document rationale if not addressed.
• Ensure safety and effectiveness of OT service delivery.
• Complete documentation timely and accurately.
• Supervise home health aide when RN is not active in the case.
• Perform other duties as assigned.
