Job summary

Adara Home Health in Minnesota seeks an Occupational Therapist Case Manager to deliver skilled home-based therapy per the Plan of Care. You will travel to clients’ homes for 1–3 hour visits, Monday through Friday, operating from the Buffalo, MN office to service Rogers and nearby communities.

You will evaluate progress, develop treatment plans, supervise therapy staff, and maintain thorough documentation while ensuring safety and confidentiality.

Qualifications

• Licensed as an Occupational Therapist in Minnesota with no restrictions.
• Graduate of an AOTA-approved school of occupational therapy and currently certified to practice.

Responsibilities

• Perform initial and periodic assessments to determine care needs and outcomes.
• Provide OT treatments, evaluations and prescribed activities.
• Supervise COTAs, HHAs, LPNs, and RNs in therapy programs.
• Document progress notes and communicate with physicians and managers.
• Educate families and provide home therapy guidance.
