Job summary

CenterWell Home Health is seeking a licensed Occupational Therapy Assistant to join our Home Health Branch Administration team. You will assist the supervising OT to deliver rehabilitation services in patients' homes, helping restore strength, mobility and independence.

This role includes designing/adapting activities, documenting progress, coordinating care with the interdisciplinary team, and supporting patients and families in home programs.

Qualifications

• Minimum six months occupational therapist assistant experience
• Current OTA licensure required
• Valid CPR certification preferred

Responsibilities

• Provide therapy services planned, delegated and supervised by the qualified Occupational Therapist as per Plan of Treatment
• Assist in implementing vocational/education programs established by OT
• Design/adapt equipment and living environments
• Fabricate devices to assist function and independence
• Provide therapeutic treatment and instruction to patients per physician orders
• Record observations in patient records and prepare progress notes
• Instruct patients and family members on home programs and use of adaptive equipment
• Participate in care coordination and discharge planning with the interdisciplinary team
• Design community reintegration activities to assist patient physical reconditioning and adjustment
