Job summary

Humana's CenterWell Home Health is seeking an Occupational Therapist to deliver rehab in patients' homes, developing retraining plans and coordinating with care teams. You will assess ADLs, mobility, and function, and guide patients toward independence with targeted interventions and family education.

The role emphasizes collaboration with physicians, adherence to Plan of Care, and documenting patient progress.

Qualifications

• Current and unrestricted OT licensure.
• Current CPR certification.
• Valid driver's license, auto insurance and reliable transportation.
• Preferred: six months of occupational therapy experience.
• Preferred: Home Health experience.

Responsibilities

• Assess daily living/work-related skills and develop retraining programs.
• Confer with physician and care team to update the Plan of Care.
• Provide therapy to improve/restore strength, ROM and function.
• Review retraining programs in line with patient goals and rehab program.
• Coordinate with vocational team and implement rehab plans.
• Document patient status, progress and care plans with the physician.
• Participate in care coordination and discharge planning.
• Design community reintegration activities and adapt home/work environments.
• Design training for staff and family on retraining activities.
• Supervise OT Assistant and Home Health Aide performance.
