Job summary

CenterWell Home Health, part of Humana Inc., is seeking a Home Health Occupational Therapist to assess, retrain and assist patients in regaining independence in their homes. You will work with the physician and care team to implement plans of care and coordinate discharge planning.

Responsibilities include administering tests, designing retraining programs, and instructing staff and family to carry out therapy goals. Includes driving to patient homes and meeting a 32-hour weekly schedule.

Qualifications

• Current and unrestricted OT licensure.
• Current CPR certification.
• Valid driver's license, auto insurance and reliable transportation.

Responsibilities

• Assess/screen patient's daily living/work-related skills and develop therapeutic retraining programs with measurable objectives.
• Administer and interpret diagnostic and prognostic tests of function in patient homes to assist physician evaluation.
• Confer with physician and care team to develop/revise the Plan of Care.
• Provide therapeutic treatment to improve/restore strength, coordination and function per physician orders.
• Review retraining programs aligned with behavioral goals within rehab guidelines.
• Consult with vocational team to develop vocationally oriented plans.
• Report patient status to physician, rehabilitation staff and family.
