Job summary

Humana CenterWell Home Health is seeking a licensed Occupational Therapist to deliver home-based rehabilitation, assess function, and develop retraining plans for patients in their homes and communities. You will coordinate with physicians and the care team to implement plans that improve strength, mobility and independence.

Responsibilities include documenting progress, participating in discharge planning, and training staff and family to support ongoing rehabilitation.

Qualifications

• Current and unrestricted OT licensure.
• Current CPR certification.
• Valid driver's license, auto insurance and reliable transportation.

Responsibilities

• Assess/p screen patient’s daily living or work-related skills and develop retraining programs with measurable objectives.
• Administer and interpret diagnostic tests of function to patients in their homes to help evaluate function.
• Confer with the patient’s physician and care team and participate in updating the Plan of Care.
