Qualifications

• Degree from an accredited Occupational Therapy Program.
• Six months of OT experience preferred.
• Home Health experience a plus.
• Current and unrestricted OT licensure.
• Current CPR certification.

• Valid driver’s license, auto insurance and reliable transportation.

Responsibilities

• Assess and screen patients' daily living and home-rehab needs with measurable goals.
• Deliver in-home occupational therapy to improve strength, range of motion and function.
• Collaborate with physicians and rehab team to update the Plan of Care.
• Document care observations and progress in patient records daily.
• Coordinate with family and other health professionals to support home health goals.
• Develop and train caregivers in retraining programs and safety techniques.
• Assist with discharge planning and community reintegration efforts.
