Job summary

Humana Inc. through CenterWell Home Health seeks a Home Health Occupational Therapist to evaluate and treat patients in home settings, coordinating with physicians and the rehab team to improve independence and quality of life.

You will assess daily living skills, design retraining plans, and document progress while collaborating on care plans and discharge planning for patients' rehabilitation journeys.

Qualifications

• Current and unrestricted OT licensure.
• Current CPR certification.
• Valid driver's license, auto insurance and reliable transportation.

Responsibilities

• Assess and screen patients' daily living and work-related skills; develop retraining programs with measurable objectives.
• Administer and interpret tests of function to support physician evaluation of function.
• Confer with physicians and the care team to update the Plan of Care and rehabilitation goals.
• Provide therapeutic treatment to improve/restore strength, coordination, ROM and function.
• Document patient progress and care observations in a timely manner.
• Participate in care coordination, discharge planning and family education.
