Job summary

Commonwealth Home Health seeks an Occupational Therapist to assess and address functional status, ADLs, and home safety needs in patients, coordinating with the health care team across CMS requirements.

The role includes evaluating functional status, developing therapy plans with physicians, and documenting patient progress. Eligible candidates should hold an OT licensure, have a valid driver's license, and be prepared to work in patients' homes with strong communication skills.

Qualifications

• Current Occupational Therapy licensure in state of practice.
• Current CPR certification or ability to obtain within 90 days of hire.
• Good communication, writing, and organizational skills.

Responsibilities

• Provides service within the scope of practice as defined by state laws governing Occupational Therapy and in accordance with the plan of care.
• Evaluates patient functional status and CMS documentation requirements; consults with physician on therapy plan.
• Observes, records, and reports patient responses and changes to condition to nurse supervisor and physician.
• Conducts patient assessments to evaluate functional level using functional tests.
• Ability to work in any home setting regardless of age, race, creed, or disability.
