Qualifications

• Graduate of an accredited OT program.
• Active, unencumbered OT license in the state of practice.
• Current CPR certification by an approved provider.
• Preferred: one year of clinical experience in home health or related setting.
• Continuing Education requirements as outlined by policy, including OSHA/ Infection Control.

Responsibilities

• Assess and plan: conduct evaluations of patient functional abilities and create individualized treatment plans.
• Implement interventions: provide OT treatments to improve self-care, mobility, and adaptive skills.
• Educate and support: train patients and caregivers on therapeutic activities, safety, and adaptive equipment use.
• Collaborate with care team: communicate with physicians, nurses, and other professionals for coordinated care.
• Document accurately: complete timely documentation of evaluations, treatments, and progress.
• Assist with equipment: recommend and train on assistive devices and adaptive equipment.
• Participate in meetings: attend care conferences, in-services, and team meetings as required.
• Maintain competency: continue OT skills and comply with state licensure and CE requirements.
