Job summary

Qualifications

• Current and unrestricted OT licensure.
• Current CPR certification.
• Valid driver's license, auto insurance and reliable transportation.
• Preferred: Six months of occupational therapy experience in home health.
• Preferred: Home Health experience.

Responsibilities

• Assess/screen patients' daily living/work-related skills and develop therapeutic retraining programs.
• Administer and interpret functional tests at patients' homes to aid physician assessment.
• Collaborate with the physician and care team to plan and revise the care plan.
• Provide therapy to improve/restore strength, coordination, ROM and function.
• Document patient progress and discharge summaries accurately.
