Job summary

Olympia Assured Home Health is seeking an Occupational Therapist to assess and address patients’ functional status and daily living activities in home health settings. You will evaluate needs, develop therapy plans, and coordinate with physicians and nurses to optimize care.

Responsibilities include implementing evidence-based OT interventions, maintaining progress documentation, and collaborating with the care team.

Qualifications

• Current Occupational Therapy licensure in state of practice.
• Valid driver’s license and vehicle insurance or access to dependable transportation.
• Ability to work in diverse home environments regardless of client characteristics.

Responsibilities

• Assess and evaluate patient care needs related to functional status and activities of daily living.
• Plan and implement occupational therapy interventions and adaptive equipment recommendations.
• Collaborate with the health care team to develop and modify the therapy plan of care.
• Document patient progress and communicate changes to the nurse supervisor and physician.
