Assured Home Health seeks an Occupational Therapist to provide home health services in Washington state. You will assess functional status, ADLs, fine motor skills, and home safety, coordinating with physicians to develop and implement therapy plans.

Responsibilities include evaluating patient progress, documenting CMS-related status, and recommending adaptive equipment. A valid WA OT license and CPR certification are required.

Qualifications

• Licensure to practice Occupational Therapy in Washington state.
• Current CPR certification required.
• Valid driver’s license and access to a dependable vehicle or transportation.

Responsabilités

• Assess and evaluate patient care needs related to functional status and daily living activities.
• Coordinate with physicians and health care team for therapy plan of care.
• Document patient progress and response to treatment for CMS requirements.
• Conduct home assessments and recommend adaptive equipment as needed.
