Job summary

Assured Home Health, part of the Optum family, is seeking an Occupational Therapist in Home Health to assess and address functional needs in patients' homes. You will evaluate daily living activities, coordinate with the care team, and ensure plans of care are followed with quality documentation.

Qualified candidates hold Washington OT licensure, have a valid driver's license, and may obtain CPR certification if not currently held. A sign-on bonus is offered for eligible full-time hires.

Qualifications

• Current Occupational Therapy licensure in Washington is required.
• Valid driver's license and reliable transportation or access to public transit.
• CPR certification preferred or obtainable within 90 days.

Responsibilities

• Assess and evaluate patient care needs related to functional status and daily living activities.
• Perform home assessments and recommend adaptive equipment as needed.
• Coordinate with physicians and care team to develop therapy plans.
• Document patient progress and respond to changes in condition.
• Provide care with respect for all patients regardless of background.
