Job summary

Providence at Home with Compassus is seeking a Home Health Occupational Therapist to model Compassus values and deliver coordinated care across age groups in a Home Health setting.

The role involves planning, implementing, and evaluating patient care plans, performing OASIS assessments, and collaborating with the IDT to restore or maintain patient well-being. Active OT license and CPR are required.

Qualifications

• Bachelor of Science in Occupational Therapy required.
• Two years of experience as an Occupational Therapist in an acute care or rehabilitation setting highly preferred.
• Active and unencumbered Occupational Therapist license in state(s) of employment required.
• Current CPR certification required.

Responsibilities

• Provides care utilizing infection control measures that protect both the staff and the patient (OSHA).
• Demonstrates commitment, professional growth, and competency; attends in-services as required by regulation.
• Accepts clinical assignments consistent with education and competence to care for patients.
• Monitors assigned cases to ensure compliance with third party payor requirements.
• Provides effective communication to patient/family and team members to ensure coordination of services.
• Performs initial and ongoing clinical assessments (Agency/OASIS) and evaluates sensory-motor, psychosocial, and cognitive-perceptual components of occupational behavior.
• Assesses physical, social, safety, and cultural components of the patient’s occupational environment.
• Teaches client/family disease process and treatments needed.
• Fabricates and maintains adaptive equipment and hand splints.
• Documents and completes a Plan of Care, progress notes, phone orders, and other clinical documentation; plans for discharge.
