Qualifications

• Active OT license in state(s) of employment.
• At least 1 year experience in acute care, home health, or rehab preferred.

Responsibilities

• Develop OT plans of care and conduct home visits for assessments.
• Implement plans of care and document visit notes in a timely manner.
• Educate patients, families, and team members on treatment plans and safety.
