Job summary

Providence at Home with Compassus seeks an Occupational Therapist to evaluate patients in the home, develop a plan of care, implement therapy, and perform discharge planning as part of a multidisciplinary Hospice team.

You will educate patients and families, teach home exercise programs, document care, and coordinate with IDG members to ensure safe functioning at home and optimal outcomes.

Qualifications

• Required: 1 year experience in acute care, rehabilitation, or home care.
• Preferred: Coursework in Treatment of Adult Hemiplegia and Hand Therapy.
• Home care experience preferred.

Responsibilities

• Evaluate physical, cognitive, and functional status to determine home safety and caregiver needs.
• Develop and communicate a plan of care with the Hospice team; follow physician orders.
• Provide patient/family education and counseling about treatments and goals.
• Teach home exercise programs and compensatory techniques for home management.
• Document assessments and communications accurately and timely.
