Job summary

Qualifications

• Required: 1 year experience in acute care, rehabilitation, or home care.
• Preferred: Home care experience.
• Preferred: Training in treatment of adult hemiplegia and hand therapy.

Responsibilities

• Evaluate patient status to determine home safety and functional capabilities.
• Develop a plan of care in collaboration with patients/families and Hospice team.
• Provide patient/family education and counseling on treatments and goals.
• Teach home exercise programs and compensatory techniques.
• Provide case management and coordinate services per care plan.
• Document assessments, interventions, and communications accurately.
