Job summary

Best Home Health Providers Inc. is seeking an Occupational Therapist to support patients in their homes across the Bay Area and beyond. The role focuses on helping patients regain independence, improve function, and navigate daily life safely from their own homes.

This per diem position offers a flexible schedule, opportunities to convert to full-time based on performance, and a competitive per-visit rate of $110–$120. CA licensure and CPR are required, with travel to patient homes expected.

Qualifications

• Graduate of an occupational therapy curriculum accredited jointly by the AMA Committee on Allied Health Education and Accreditation and the AOTA
• Registered with the National Registration Examination of the American Occupational Therapy Association
• Active California Occupational Therapy license
• Minimum 2 years of clinical OT experience
• Current CPR certification with AHA
• Valid driver’s license, reliable insured vehicle, and ability to travel to patient homes

Responsibilities

• Evaluate patients’ functional abilities and home environments to identify needs and safety concerns
• Develop personalized treatment plans and communicate them to physicians and the care team
• Teach and retrain patients in daily living skills affected by illness or injury
• Recommend and instruct patients on adaptive equipment, splints, and prosthetic/orthotic devices
• Maintain accurate clinical records and promptly report changes in patient condition
• Coordinate with the broader care team to ensure seamless, patient-centered care
• Educate family members and caregivers on the patient’s treatment approach
• Supervise OT Assistants and oversee home health aide care when OT is the sole skilled service
• Support discharge planning as patients transition out of home health care
