Qualifications

• Graduate of an accredited OT curriculum and eligible for state licensure.
• Current OT registration/licensure as required by state(s) of practice.
• Minimum 1 year experience in home health or equivalent.
• CPR certification.
• Strong verbal and written communication skills.
• Licensed driver with reliable transportation.

Responsibilities

• Performs occupational therapy assessments, diagnostic tests, skilled treatments, and ongoing evaluation of clients.
• Develops treatment programs and establishes goals for improved function.
• Documents findings, plans, and progress in designated forms and reports.
• Instructs clients and families in home-exercise programs and adaptive strategies.
• Communicates plans to physicians, Case Manager, and other staff; participates in care conferences.
• Performs care coordination and interdisciplinary referrals as needed.
