Qualifications

• Graduate of an accredited occupational therapy curriculum and eligible for or licensed in state(s) of practice.
• Minimum of 1 year experience in home health or equivalent.
• CPR certification and valid driver's license with reliable transportation.

Responsibilities

• Performs assessments, develops treatment programs, and establishes goals for client function.
• Documents findings, plans, and progress; communicates with physician and care team.
• Provides adaptive/assistive devices and instructs client and family on therapy plans.
• Attends care conferences and participates in interdisciplinary referrals as needed.
• Ensures compliance with state, federal, and JCAHO standards.
