Job summary

Western Connecticut Home Care in Danbury is seeking an occupational therapy professional to join a home health team providing focused care at patients’ homes. You will work with physicians, patients, and families to carry out the plan of care with independence and coordination.

The role emphasizes upper extremity function, independence in activities of daily living, and cognitive-perceptual motor function, with ongoing interdisciplinary assessment and education.

Qualifications

• Minimum experience: one year.
• Experience in a qualified home health agency preferred.

Responsibilities

• Develops, implements and evaluates the plan of care in partnership with the patient, representative (if any), caregiver(s) and physician(s).
• Provides services ordered by the physician as indicated in the plan of care.
• Provides direct occupational therapy services per the plan of care.
• Maintains records on visits and clinical notes; discharge planning as applicable.
• Participates in case management conferences to coordinate the program with total care.
