Job summary

Qualifications

• Graduate of an accredited occupational therapy curriculum and eligible for state licensure.
• Current registration/licensure as required by state of practice.
• Minimum of one year experience in home health or equivalent.
• CPR certification.
• Strong verbal and written communication and interpersonal skills.
• Licensed driver with reliable transportation.

Responsibilities

• Performs occupational therapy assessments, diagnostic tests, skilled treatments, and ongoing evaluation under a medically approved Plan of Care.
• Develops a treatment program and establishes goals for improved function.
• Documents findings, plans, interventions, and client progress; communicates with physician and care team.
• Instructs clients and families in home exercise programs and safe use of assistive devices.
