Job summary

Top Echelon LLC is seeking an Occupational Therapist for Home Health. The role involves patient assessments, developing and implementing individualized care plans, and coordinating with physicians and the care team to achieve goals and positive patient experiences.

The therapist will deliver or direct most of the patient’s occupational therapy in the home health setting, ensuring documentation is timely and compliant with regulations, and promoting patient education and engagement.

Qualifications

• Bachelor of Science in Occupational Therapy from an accredited program.
• Successful completion of the NBCOT.
• State license to practice Occupational Therapy.
• Reliable transportation and valid automobile liability insurance.

Responsibilities

• Perform patient assessments and participate in individualized care planning.
• Develop and implement care plans focused on goal achievement and patient engagement.
• Coordinate with physicians and care team for continuity of care in home health.
• Ensure timely documentation per Agency policy and regulatory requirements.
