Job summary

CenterWell Home Health is seeking an in-home Occupational Therapist to help patients regain strength, mobility and independence in their own homes. You will assess, plan and implement rehabilitation programs under physician orders, coordinating with the care team to support outcomes.

You will also educate patients and families, document progress, and participate in discharge planning, with a focus on functional improvement and community reintegration.

Qualifications

• Current and unrestricted OT licensure.
• Current CPR certification.
• Valid driver's license, auto insurance and reliable transportation.

Responsibilities

• Assess/screen patients' daily living/work-related skills and develop therapeutic retraining programs with measurable objectives.
• Confer with physicians and care team to develop or revise the Plan of Care.
• Provide therapeutic treatment to improve/restore strength, coordination, ROM and function as ordered.
