Job summary

Legacy Lifecare Hospice and VNA in Massachusetts seeking a licensed Occupational Therapist (OT) to provide skilled therapy on a per-visit basis. This PRN role offers flexible scheduling and no weekend or holiday shifts, focused on helping residents maintain independence and safety.

You will evaluate residents, develop treatment plans, collaborate with the care team, educate families on ADLs and adaptive equipment, and document progress in compliance with regulations.

Qualifications

• Graduate of an accredited occupational therapy program.
• Current MA Occupational Therapist license.
• Minimum 1-year experience, preferably in a long-term care setting.
• Ability to communicate fluently in English – both verbally and written.

Responsibilities

• Evaluate residents and develop individualized treatment plans to improve independence, functional abilities, and quality of life.
• Provide skilled occupational therapy interventions to enhance ADLs, upper extremity function, mobility, balance, and safety.
• Collaborate with physicians, nursing staff, rehabilitation team members, and families to establish goals and deliver coordinated care.
• Recommend and train residents and caregivers in adaptive equipment, environmental modifications, and fall prevention strategies to promote safe, independent living.
• Complete timely, accurate documentation, monitor resident progress, adjust treatment plans as needed, and maintain compliance with regulatory and facility standards.
