Legacy Lifecare Hospice and VNA seeks a licensed Occupational Therapist (OT) to provide PRN services at $90 per visit. The role emphasizes flexible scheduling with no weekends or holidays, and requires an active MA OT license with at least 1 year of experience, preferably in long-term care.

Qualifications

• Graduate of an accredited occupational therapy program.
• Ability to communicate fluently in English – both verbally and written.

Responsibilities

• Evaluate residents and develop individualized treatment plans to improve independence, functional abilities, and quality of life.
• Provide skilled occupational therapy interventions to enhance ADLs, upper extremity function, mobility, balance, and safety.
• Collaborate with physicians, nursing staff, rehabilitation team members, and families to establish goals and deliver coordinated care.
• Recommend and train residents and caregivers in the use of adaptive equipment, environmental modifications, and fall prevention strategies to promote safe, independent living.
• Complete timely, accurate documentation, monitor resident progress, adjust treatment plans as needed, and maintain compliance with regulatory and facility standards.
